Join Our Waitlist

Submitting this form places an individual on our waitlist. We will reach out as openings become available and when placement is suitable.

 

 

Who Can Refer: Treatment centers, outpatient programs, social workers, probation officers, family members—or the individual themselves can refer.

Intake Process at a Glance:

  1. Waitlist Notification - Individuals on our waitlist are contacted by text or email when a bed becomes available.

  2. Phone Interview - We speak with the individual to confirm readiness and overall fit.

  3. IOP Intake Date and Full Application & Agreement - Accepted candidates complete the application, have IOP scheduled and agree to all house rules and program expectations.

  4. Eligibility Review - We confirm PMI number/insurance coverage, a primary Substance Use Disorder (SUD) diagnosis, an approved comprehensive assessment, and IOP enrollment (9+ hours per week).

  5. Move-In Preparation - Move-in details are coordinated once acceptance, IOP intake and bed availability align.

    Availability Note - Placement decisions are made based on availability and fit after all intake steps are completed.
